ADMISSIONS PROCEDURES FOR INCOMING FIRST YEAR IN UNDERGRADUATE PROGRAMS

 

  1. Complete the information in the system/online (www.ceu.edu.ph/ceuadmission/form)

  2. Go to Admissions Section located at office of the Registrar for printing & checking of Application Form and submission of one (1) 1.5 x 1.5 ID picture
    & grade 11 card / F138  w/ GWA

  3. Pay the entrance examination fee at the Cash Division Section.

  4. Take entrance examination at the Guidance and Counseling Section (GCS)

Registration for examination is at least 15 minutes before the chosen schedule

*Please bring a pencil with eraser during the exam

  1. Results of the exam will be sent thru email.

  2. Prepare the following requirements for admission:

  • Grade 11 Form 138/Card with general weighted average

  • Certificate of Good Moral Character from High School Principal

  • Original Form 138/Grade 12 Report Card/PEPT Result/ALSA&E Result

  • PSA copy of Birth Certificate

  1. Proceed to Admissions Officer to get/sign the following requirements for admission/enrollment:

  • Routing Form for Enrollment

  • Student’s Pledge

  •  Waiver/Informed Consent Form

Resident Aliens/ Filipino Who Studied Abroad

  1. Complete the information in the system/online (www.ceu.edu.ph/ceuadmission/form)

  2. Go to Admissions Section located at the Office of the Registrar for printing & checking of Application Form for Admission and submission of one (1) copy of 1.5 x 1.5 ID picture.

  3. Go to the Dean’s Office/Program Head’s Office for initial interview and get the referral note for drug testing and Endorsement Form for Entrance Exam at the Guidance & Counseling Section (GCD).

  4. Proceed to the Clinical Laboratory (assigned by the school) for drug test within the day of payment.

  5. Pay the Drug Test fee at the assigned Drug Testing Clinic.

  6. Claim drug test result.

  7. Pay the Entrance Examination Fee to Cash Division Section.

  8. Go to the Guidance and Counseling Section (GCS), presents referral from the dean/program head and receipt of examination fee and take the entrance examination. 

Registration for examination is at least 15 minutes before the chosen schedule

as specified in the online registration.

  1. Prepare the necessary requirements for admission:

(Filipinos Who Studied Abroad)

  • Birth Certificate with Registry Number (Photocopy)

  • Certificate of Recognition (as Filipino Citizen/ or Filipino Citizenship)

  • Original Transcript of Records

  • Photocopy of Passport

 (Resident Aliens)

  • Birth Certificate with Registry Number (Photocopy)

  • Certificate of Good Moral Character from High School Principal

  • I-Card

  • Original Transcript of Records

  • Recent 1.5 x 1.5 I.D. picture (2 copies)

11.   Go to the Office of the Registrar for the checking of requirements

12.  Proceed to Admissions Section for the following requirements for admission/enrollment:

  • Routing Form for Enrollment

  • Student’s Pledge

  • Waiver/Informed Consent Form

NON-RESIDENT ALIENS

  1. Complete the information in the system/online (www.ceu.edu.ph/ceuadmission/form)

  2. Go to the Admissions Section located at the Office of the Registrar. for printing & checking of Application Form for Admission and submission of one (1) copy of 1.5 x 1.5 ID picture.

  3. Prepare the following for submission to the Office of the Registrar: 

  • Appropriate visa,

  • Certificate of Graduation*

  • Original Transcript of Records*

  • Photocopy of passport,

  • Police clearance* (*shall be authenticated by the Philippine Consulate abroad)

  1. Go to the Dean’s Office/Program Head’s Office for initial interview and get the referral note for drug testing and Endorsement Form for Entrance Exam at the Guidance & Counseling Section (GCD).

  2. Proceed to the Clinical Laboratory (assigned by the school) for drug test within the day of payment.

  3. Pay the Drug Test fee at the assigned Drug Testing Clinic.

  4. Claim drug test result.

  5. Pay the Entrance Examination Fee to Cash Division Section.

Admissions Procedure for Transfer/ Second Degree Applicants

  1. Complete the information in the system/online (www.ceu.edu.ph/ceuadmission/form)

  2. Go to the Admissions Section located at the Office of the Registrar for printing and checking of the Application Form for Admission and submission of (1) copy of 1.5 x 1.5 ID picture.

  3. Go to the Dean’s Office/Program Head’s Office for initial interview and get the referral note for drug testing and Endorsement Form for Graduate School Admission Tests (Mental Ability Test, English Proficiency Test, Personality Test) at the Guidance & Counseling Section (GCD).

  4. Proceed to the Clinical Laboratory (assigned by the school) for drug test within the day of payment.

  5. Pay the Drug Test fee at the assigned Drug Testing Clinic.

  6. Claim drug test result.

  7. Pay the Graduate School Admission Test to Cash Dept./ Division/ Section.

  8. Proceed to GCS to take the Graduate School Admission Test and accomplish Freshmen Profile Survey.

  • Original Scholastic Records bearing School Seal and Signature of the Registrar

  • PSA copy of Birth Certificate

  • Students and Parents/ Guardians letter seeking for admission

  • Transfer Credentials

  • Certificate of Eligibility for Dentistry (for Dentistry applicants only)

(Second Degree)

  • Certificate of Good Moral Character from Authorized Official of former school

  • Entrance evaluation of subjects taken by the Dean/Program Head/ Academic Dept.Head

  • Original Scholastic Records

  • PSA copy of Birth Certificate

  • Students and Parents/ Guardians’ letter seeking admission

  • Transfer Credentials

  • Recent 1.5 x 1.5 I.D. picture (2 copies)

  1. Return to Admissions Officer to get/sign the following requirements for admission/enrollment:

  • Routing Form for Enrollment

  • Student’s Pledge

  • Waiver/Informed Consent Form

Requirements for Application For Filipino Applicants:

  1. 1.5 x 1.5 inches ID Picture (2 pcs.)
  2. Filled out Application for Admission Form

    
For Foreign Student Applicants:

(Resident Alien)

  1. I-card
  2. 1.5 x 1.5 inches ID Picture (2 pcs.)
  3. Filled out Application for Admission Form

(Non-resident Alien)

  1. Passport size ID Picture (2 pcs.)
  2. Police Clearance
  3. Photocopy of Passport
  4. Filled out Application for Admission Form

 

 

Requirements for Enrollment

  1. Original Form 138/High School Report Card
  2. Good Character Testimony from the high school principal
  3. NSO Birth Certificate (original and photocopy)
  4. Filled out Application for Admission Form
  5. 1.5 x 1.5 inches I.D. picture (2 pcs.)

ADMISSIONS PROCEDURE FOR GRADUATE SCHOOL

  1. Complete information in the system/online (www.ceu.edu.ph/ceuadmission/form)
  2. Go to Admissions Section located at office of the Registrar for printing & checking of Application Form and submission of one (1) 1.5 x 1.5 ID picture.
  3. Prepare the following requirements:
    • Original Transcript of Records with S.O. number
    • Transfer Credential (If graduate from other school)
    • for female photocopy of PSA Birth Certificate/Marriage Contract.

 

  1. Proceed to the Dean’s Office/Program Head’s Office for initial interview and get the referral note for drug testing and Endorsement Form for Graduate School Admission Tests (Mental Ability Test, English Proficiency Test, Personality Test) at the Guidance & Counseling Section (GCD).
  2. With clean drug test result, proceed to Admission Test at Guidance & Counseling Section.
  3. Pay the Graduate School Admission Test to Cash Dept./ Division/ Section.
  4. Proceed to GCS to take the Graduate School Admission Test and accomplish Freshmen Profile Survey.
  5. Proceed to the GCS for the interpretation of the test result.

Registration for examination is at least 15 minutes before the chosen schedule
